A practical look at GP Patient Survey 2025—why cohort benchmarking beats national averages, what good looks like in your context, plus the ICS leaders list.

When the GP Patient Survey (GPPS) lands each year, it’s tempting to jump straight to the national average and ask, “How do we stack up?” The problem is that a national mean flattens the rich, messy reality of general practice. A single-site rural surgery serving an older population has a different context to a multi-site city practice. National numbers are a benchmark, not a guide.

This year, we’re celebrating practices that set the pace within their true peer groups - practices whose 2024 context looked like yours, and who went on to lead their 2025 cohort for overall satisfaction. These are the “apples to apples” comparisons that drive improvement.

  • Learn from leaders: see who led that cohort in 2025—and what they did differently.

  • Set credible targets: anchor goals to peer performance, not the national average.

Why broad benchmarks can mislead

Averages wash away the factors that shape performance and patient experience:

  • Practice size and capacity: Operating at 5,000 vs 20,000 patients is a different world.

  • Population and need: Age, deprivation, and local health patterns drive demand, access, and expectations.

  • Geography: Inner-city access realities aren’t the same as rural or town/fringe.

  • Starting point: A practice improving from 65% faces different trade-offs than one nudging from 85%.

Chasing a single “national average” can feel irrelevant or unachievable. Cohort-based targets, by contrast, are credible and motivating: if they can do it in our context, so can we.

How to turn GPPS into a roadmap

  1. Build your peer cohort. Identify 30–40 practices that look like you—list size, age profile, geography, and 2024 baseline performance. That gives you a fair range, from median to top quartile, for your reality.

  2. Find the drivers. Within that cohort, map the patterns that separate leaders from the rest: access channels (phone/online/in-person), booking journey, consultation experience, and the questions that most strongly correlate with overall satisfaction. This shows where a marginal gain will move the headline.

  3. Set evidence-based targets. Ditch the generic averages. Anchor your goals to what similar practices already achieve.

For example: “We rank 25th among 35 lookalike practices. The top five score 78–82%; their biggest lift is phone access. Let’s get to the 74% peer median this year by fixing that channel first.”
